9 month antiquated near disorientation of 102.3 that wont budge down?

My son is 9 months old with a fever of 102.3 and tylenol wont even budge the disorientation. he also has a bad cough and runny nose. hes unbelievably tired and doesnt want to do anything but lay on my shoulder. so what should i do in regards to seein a doctor or home care?

also i already enjoy him just in his diaper and a fan on when he is sleeping.
